Ongoing changes within Aberdeenshire Health & Social Care Partnership urgent care project mean we are in a position to create a dynamic team providing unscheduled care within our communities. The role of the Advanced Clinical Practitioner is a diverse clinical role working within Aberdeenshire. The main aim of the role is to provide positive outcomes for patients by providing enhanced healthcare assessment capacity which will meet urgent/on the day home visit requirements in the community setting. You will support the medical staff and other health care professionals in ensuring continuity and efficiency in service delivery. Interested individuals should be a registered healthcare professional with existing broad skills in healthcare assessment. Evidence of efforts to advance practice within your current role is essential. We welcome applications from District Nurses, Paramedics, or Advanced Nurse Practitioners. You must be able to evidence a good understanding of the range of conditions that may require patients to be admitted to hospital or the virtual community ward. You should be proactive in expediting the patient journey and developing the role of the Advanced Clinical Practitioner within Aberdeenshire as part of multidisciplinary team collaboration. It is essential that the individual can recognise and initiate clinical management of the deteriorating patient. You will be required to complete the specified education for the role if not already fully qualified. Appointment to the Trainee Advanced Nurse Practitioner post would be made under Annexe 21, with candidates commencing at Band 6 level until reaching the required competencies. Hours of work will be flexible to meet service needs. A valid driving licence and/or access to a car is essential. Successful applicants will use their own car for business travel.
