Job Description

London Borough of Southwark are seeking to recruit a Qualified Social Worker to join a well-managed team in Mental Health: Move On Support Team. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels.

The Council pays close attention to looking after their Social Workers and strive to ensure they feel supported and welcomed in their roles. The responsibilities of the Social Worker includes:

  • To provide a social work service within the Move On Support Team (MOST) to adults with mental health needs who live in various supported or residential accommodation.
  • To provide comprehensive and holistic person-centred assessments and specialist interventions to enable people to maximise their wellbeing, independence, choice and control regarding their accommodation and support needs.
  • MOST are a team of Social Workers and Occupational Therapists working alongside our health colleagues and with external providers. Working in collaboration, as part of the assessment and review process, you will ensure that current support plans are personalised, recovery focused and promote independence. Part of the role is to establish if service users are in the most appropriate placements to meet their needs or if an alternative to this should be sought.
  • We have the scope to facilitate creative and innovative approaches to ensure service users are at the centre of the support they receive.
  • The team's remit is expanding to work with people in supported housing and placements in Southwark and beyond, including forensic placements both in and out of borough.

To ideal candidate will have:

  • HCPC registration
  • Eligibility to work in the UK
  • Social Work qualified (MA/BSc/DPSW/CQSW/MSc/BA)
